Lootrules
Following the intial drafting of this post by Lleu, there's been a few changes in how Blizzard create loot, and how loot priority is assessed, not to mention dual specs being introduced! We still apply the same core principles to looting that we always have, and these are outlined below.

General
During raids, we divide the loot according to class and role. This means we might have up to 4 different 'phases' of rolling:
1. Need for main spec / priority classes (*)
2. Need for main spec / secondary classes (*)
3. Offspec
4. DE/Greed
Whether an item is suitable for a class/role is solely determined by the following:
- plate/mail/leather/cloth
- primary stat, i.e. str, agi, int, dodge, def
- primary weapon, i.e. ranged has prio on ranged weapons, melee prio on melee (but keep stats on weapon in account)
Players can roll if they think that the item is suitable for them and an upgrade. The ML will only interfere if a player makes a clear misjudgement (either out of ignorance or greed)
(*) As players will only be allowed to come on an alt/offspec to make the raid feasible and upon request, players who do so will have 'main' rolling rights on that character for that run. In addition, if they come on their offspec, they will have rolling rights for their original main spec as well.
10 seconds to roll
The masterlooter will call the rolls according to priority classes. When your class is called for and want to roll need for that item, do so within 10 seconds or say pass. Please drop your roll right away; waiting to see if you can outroll someone or consider rather giving the item to someone else is nice, but it usually takes an unnecessary amount of time. If you're not sure the item is an upgrade, yell "WAIT" in raidchat. This way, the masterlooter will extend the rolltime. Also, it takes an unnecassery amount of time to wait for people to say pass. Additional to the 10 seconds rule, this means when you stay silent during the roll, you automatically pass. So:
1. Drop your roll or pass within 10 seconds
2. Yell "WAIT" in raidchat if you're not sure the item is an upgrade
3. Silence means passing
The item that came along is ideal for my main spec, but I had to come on my offspec so that the raid could go ahead
On occassion, we have to ask people to use their secondary spec in order to fill out a gap in the raid composition. If the raid leader or organiser of the raid has requested that you come on your offspec to let the raid go ahead, then you can roll on the item as if you were attending on the mainspec. For example, you may normally be DPS, but were asked to come on your tank spec. A DPS item drops, and it would be an upgrade for your main spec, so you can drop a roll during the first wave of rolls.
But what if I asked to come on my offspec?
If you requested to come on your offspec, and you've been assigned a place in the raid, then it will be because we needed you to come on that spec. Therefore the same rules apply as above, you can still roll on your mainspec items. However, if something drops that you could use for your offspec, out of courtesy to your fellow raiders we would ask you to consider if you really intend to use that item, or if it would be better in the hands of someone on their main spec.
Some little facts to make masterlooting even easier
1. During the lootphase, move all the talk that doesn't have to do with rolling to /say. Especially with the 10 seconds rule where people are yelling "WAIT" in /raidchat, this is extremely important. So please move your praises about the good fight, congratulations, etc. to /say 
2. It's also easy when you know in advance if the boss we downed could drop loot you might need. Now that the Dungeon Journal has been implemented, this is even easier than ever, although there are still bosses which might have additional items which don't show in there. For people who don't put that much interest in that kind of stuff; you can always yell "WAIT" in /raidchat to buy yourself some extra time and compare the item to what you currently have.
3. During the lootphase, you can take a peek at what the boss has dropped while the ML gets on with the rolls (with any treasure chest type drops being the exception
). This way you can see what has dropped before the ML announces it and decide for yourself whether you'd like to have it or not.
4. During the lootphase don't whisper the masterlooter, and try to leave discussions about the looting procedure until after the raid.
5. During the lootphase, don't go to the toilet or get a drink; the raidleader will schedule those times. You could miss out on valuable offspec loot or info, not to mention fun.

Two things I like to add
Two things I like to add from my own experience as Master Looter:
1) Please do not roll on an item *before* the Master Looter has typed the roll call for that item in raid chat. And certainly do not roll twice. Doing so can easily lead to confusion and interference with fair loot distribution.
2) Most of the time Master Looters use the ClassLoot addon (which is an electronic version of wow-loot.com) to decide the correct roll order. If you install this addon yourself, you can easily see for which classes and in which order the Master Looter will do the roll calls.

Loot Rules Update - 2010
All of the above still applies, however with the advent of dual spec and more of us having ALTs that are able to raid, we felt the need to codify some elements of Loot Rules that we already practice.
I am attending a raid on an ALT (main spec)
You can roll within reason, and only with ML approval* (especially if there is a raider on his/her main toon and main spec that needs said item) on gear for your ALT (main spec).
I am attending a raid on an ALT (secondary spec)
You can roll within reason, and only with ML approval* (especially if there is a raider on his/her main toon and main spec that needs said item) on gear for your ALT (main spec).
You can roll on gear for your ALT (secondary spec), but only if there is no raider on his main toon and main spec that requires said item BUT you will have priority over all other persons rolling for offspec.
I am attending a raid on my MAIN (secondary spec)
You can roll on gear for your MAIN (main spec) gear in the usual manner.
You can roll on gear for your MAIN (secondary spec) gear only if there is no raider on his main toon and main spec that requires said item BUT you will have priority over all other persons rolling for offspec.
* - this responsibility should really rest only with an experienced ML (for the most part, an officer) ... so in some cases this may be a decision for "the Loot Council" a.k.a the officers online at the time!

Did I miss something?
I am reading through your rules and found here something I am not familiar with. What are priority classes, secundary classes, tertiary classes ? Or did I miss that in another guide?

The priority of classes is
The priority of classes is based on how much his/her class needs the loot.
A hunter would need a bow more than a rogue so the hunter gets priority over the rogue.
Hunter is here a primairy class, and the rogue the secondairy.
Same goes for leather loot. The rogue gets there the priority over hunter.
There the rogue is primairy and the hunter secondary.

Classloot
A simple way to determine this inside a raid is by using the adon classloot. It puts up information in the items tooltip which displays priority classes and secondary classes.
Primary classes are displayed with 5 stars, secundary 4 and if more with 3 and so on.
So in the bow example above, you would see 5 stars with Hunter behind it and 4 stars with rogue behind it.

Aha! That explains the 3 categories. For the most part I understand how the list is made, but there are a few weird choices - it seems to me that spirit gear should diverse more while mp5 (which is rather useless in raids) should diverse less between classes. But I understand the system now, it makes sense to use this as general ruler. Thanks for explaining.

Well, the masterlooter (mostly an guild officer) is the one who makes final decisions regarding loot, but most of this time it works out well with this system.
And well, if you eventually run ICC a lot, most primary classes have it already and secondary (or even offspec) can drop a roll. Or for example a Warrior rolling on leather because of better stats than the plate equivalent, if it otherwise would be DEed.

The main thing is that I want to have a stable ruleset in place, which the site provides. It's not always perfect but it helps keep things in balance
